One way to hold this word: made-up is what something looks like after a person has had a hand in it. It was not simply there. Somebody produced it — out of their head, out of a make-up box, out of a pile of parts, out of a load of soil. So the word never describes the raw thing; it describes the result, with a person behind it. Which sense you are reading comes from what is being described: a story, a face, a bed, a piece of land, or a person.
