Picture a vertical scale with a marker slid far up it. What the scale measures changes from use to use — how much of a quality there is, how far someone has been trained, how far up an organisation they sit, how well you rate them — but the marker is always near the top. One warning worth carrying: this is not a general-purpose 'very'. English only lets it sit in front of certain words.
