You can think of skewer as one long pointed stick going straight through something and out the other side. Once it is through, nothing can slide off and nothing can wriggle away: the pieces are lined up on that one line, held exactly where the point put them. Both parts of that picture matter later, the going through and the being held, because the other uses of the word take one or the other and run with it.
