Picture the moment the smiling stops: whatever is being said or done now carries weight, so it is not to be laughed off. That weight is what the adverb adds. Point it at a manner and you get 'not joking'; point it at a degree and you get 'badly, to a worrying extent'; put it in front of a sentence and it warns the listener that the joking part is over.

可以想成：笑容收起來的那一刻——現在講的、做的這件事有份量，不能當笑話帶過。這個副詞加上去的就是那份重量。把它指向做事的態度，得到的是「不是在開玩笑」；指向程度，得到的是「嚴重到讓人擔心」；放在一句話前面，它等於先提醒對方：玩笑到此為止。
