One way to hold this word: the back of your throat is a short pipe with liquid standing in it. You tip your head back so the liquid stays there, then push air up from your lungs. The air has to force its way through, and that is where the rough bubbling noise comes from. Everything else the word does is one piece of that picture, picked up and used on its own — the liquid standing in the pipe, the single go at it, or just the noise.

English took the word from French gargouiller, 'to gargle', which goes back to Old French gargouille / gargole, meaning 'gutter, throat' — the same word behind gargoyle, the carved waterspout on a church. So a pipe with water running noisily through it is not a later metaphor here; it is what the word was built on.
