Think of a path worn across a patch of grass. Nobody planned it; people simply walked the same way often enough that it became the way. Accustomed is that worn path in word form — either the person who no longer notices the walk, or the route that is simply the usual one.

The source entry analyses the word as accustom + -ed, and accustom itself carries custom inside it. That fits the picture: something has been made into a custom, so it now simply is what happens.
