Picture warm mineral water and people coming to sit in it because they believe it will do them good. That is the oldest picture in the word: a place you travel to for the water. As the water stayed but the reason — health, beauty, relaxing — the word followed, and in American English it shrank all the way down to the hot tub in someone's back garden.
