A life-threatening illness, injury, or situation is one that could cause a person to die. Doctors use it with care: a bad cold is not life-threatening; a heart attack may be. News reports use it for accidents, storms, and disease. It is serious, ordinary-to-formal English, not a joke. Though actually “could cause death” is the test, not “feels very scary.” Many painful things are not life-threatening, and a quiet problem — internal bleeding, a hidden allergy — can be. Before a noun, the is the normal spelling: a life-threatening injury.
