Picture someone who has dropped out of ordinary life for a while: the bed is unmade at two in the afternoon, work is on hold, someone else is doing the shopping. Illness can be thought of as that whole state of not-being-well — the period a person is in, rather than the or the damage itself. That is why English says a long illness, a serious illness, off work with an illness: it is measured in time and in how much of your life it takes over.

可以想成一個人暫時從正常生活裡「退出來」的樣子：下午兩點床還沒整理，工作先停著，買菜的事交給別人。illness 的畫面比較像這整段「不舒服、不能照常過日子」的狀態，而不是那隻病菌或身體壞掉的那個點。所以英文才會講 a long illness（一場久病）、a serious illness（重病）、off work with an illness（因病請假）——它是用時間長短、用佔掉多少生活來衡量的。跟它常放一起的 disease 比較像是「有名字的那個病」，illness 比較像是「人生病的那段狀態」，中文兩個都翻成「疾病」，但語感上有這層差別。
