Gruffly tells you how someone speaks: in a rough, short, unfriendly-sounding voice. “He answered gruffly” means the words came out low and blunt, not warm. As with gruff, this often describes the surface, not the heart. A teacher might say gruffly “Sit down” and still care about the class. You will see it in stories and reported speech more than in student essays.
