You can picture always as covering the whole stretch of time with no gaps — at every single point along the line, the thing is true. Originally it even pointed at distance: 'all the way' along a road, every step of it; that picture later slid onto time.

From Old English 'ealne weg' (literally 'all the way', accusative of distance), a mid-14c. contraction of all + way; the -s adverbial ending was added later. The 'every step of the whole way' image underlies the modern 'at every point in time'.
